IDPH Health Requirements for Enrollment
2024-25
Note: All health records must be submitted to the School Office PRIOR to the first day of school, unless otherwise noted.
All exams must be complete and recorded on State of Illinois Examination forms only. Completed forms may be faxed to FBCS at (309) 347-8716.
Authorized forms are available on the Illinois State Board of Education websites (links follow at the end of this document).
Failure to comply with the State of Illinois regulations will result in exclusion from school beginning October 16.
BIRTH CERTIFICATE REQUIREMENT:
ILLINOIS LAW REQUIRES that within 30 days, every person enrolling a child in school must provide either a certified copy of the child’s birth certificate or other reliable proof of the child’s identity and age and an affidavit explaining the inability to produce a copy of the birth certificate.
Other reliable proof shall include a passport, visa or other governmental documentation of the child’s identity.
The school shall promptly make a copy of the certified copy for its record and return the original certified copy to the person enrolling the child.
SCHOOL HEALTH REQUIREMENTS
NEW PRESCHOOLERS
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history.
KINDERGARTEN
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history.
Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school.
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
SECOND GRADE
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
SIXTH GRADE
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history.
Proof of one dose of the Tdap vaccine and one dose of the Meningococcal vaccine after 11 years of age.
NINTH GRADE
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history.
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
TWELFTH GRADE
All 12th graders must have received two doses of the Meningococcal vaccine unless the first dose was administered after age 16. In this case only one dose after age 16 is required.
ALL NEW STUDENTS FROM OUT OF STATE
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history. All exams must be recorded on State of Illinois Examination forms only.
Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school.
STUDENTS TRANSFERRING FROM OTHER ILLINOIS SCHOOLS
Transfer of current Illinois school health records (This includes: physical exam with up-to-date immunizations, dental exam, and eye exam.)
HOMESCHOOLED STUDENTS
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history. All exams must be recorded on State of Illinois Examination forms only.
Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school.
SPORTS PHYSICAL

If your child will have an Illinois child health examination form completed their sports participation approval can be marked on that by the doctor.
If your child does not require an Illinois child health examination form, the following link will take you to the IHSA Pre-participation form. This should be used for their sports physical.
